Confidential Patient Information Reminders
Protected Health Information (PHI) consists of all information created or received about an individual relating to their past, present, and future physical or mental condition, as well as all data that is related to the payment for the provision of health care. Only individuals with a "need to know" should have access to patients protected health information.

Protecting Patient Privacy: Rules for Fax Machines
- Be sure to have the correct fax number and enter the fax number correctly.
- Faxes containing PHI should always include a COVER SHEET with a confidentiality notice requesting that faxes sent to the incorrect destination be destroyed, and also requesting notification to the sender of such errors.
- Sensitive faxes should NOT be left sitting in or around the machine.
